Usually one of three things: the mounting has worked loose after years of vibration, debris has landed in the blades, or the blade assembly is out of balance from caked dust. All are quick fixes – and all get worse if ignored, because vibration loosens everything around it.
A straining motor – typically a failing start capacitor or a bearing beginning to seize. Switch it off at the wall until it is looked at: a stalled, humming motor overheats fast.
Grinding is bearings, and bearings do not heal. Caught early it may be serviceable; left long enough the honest answer becomes replacement – from $250 as a complete job. Either way you will get the real answer, not the expensive one.
That is the backdraft damper – the flap that stops outside air blowing back in. When it breaks or sticks it slaps in the wind. Fixing it stops the noise and the cold draughts that come through a fan with a failed damper.
